Our Public Health team would like to hear about your experience of Health Visiting, School Health, Child Weight Management, Special Educational Needs (SEND) Services, our brand new Family Hubs and so on. The information you provide will help us prepare a Joint Strategic Needs Assessment for Children and Young People aged 0-19 (up to 25 in the case of SEND) in Thurrock. We are completing this with the support of an NHS consultancy body called NHS Solutions for Public Health, who created the survey and will be collating responses on our behalf.

This is an important opportunity for Thurrock's children, young people, families and residents to contribute to re-designing future service provisions, promoting health and wellbeing and further reducing health inequalities in our local community. We want to have a rich and meaningful engagement and can only do this with your help.

Children, young people and their families, carers and the public who use the above services in Thurrock can complete the survey. It should only take around 15 minutes of your time. We will not ask for your name or any personal details and, although we are very interested to hear your views, please don’t feel you must answer every question.

Prize Draw

Anyone who completes the survey before its closing date can enter a prize draw for a chance to win 1 of 10 gift vouchers worth £40 each.

The survey is anonymous so if you would like to take part in the prize draw, please send an email to the email address shown at the end of the survey, telling us the date and time you completed it. In line with data protection law, the email you send will be used only for the purpose of the prize draw and will be deleted after.

Each person who completes the survey can enter the prize draw only once. Winners will be chosen at random and notified by email soon after the closing date.
