As Apprentice Social Workers (Student Social Workers), Kate and Ann-Marie have to request and receive feedback on their practice and quality of work completed during their placement within Adult Social Care (Thurrock Council). This is a requirement of their Social Work degree programme at Anglia Ruskin University.

Kate or Ann-Marie have identified that you may be willing to provide some feedback. The below form provides the student social workers with constructive feedback so they can develop and improve their social work practice.

The feedback will be anonymised, and be included in university portfolios, so please do not mention any personal details.
